request for information from Lazada E-Services Philippines,
Inc. or Shopee Pte Ltd., its affiliates, subsidiaries, officers,
employees, agents, or representatives for the purpose of
processing my loan application. I/We expressly consent to
the transfer, disclosure, or processing by Lazada Group or
Shopee Group in favor of KAYAMO, of my/our personal data
(as defined by the Data Privacy Act of 2012 and its
Implementing Rules and Regulation), seller data, transaction
information, and other information or records in the
possession of Lazada Group of Shopee Group. I/We have
been informed of the identity of the personal information
controller that will be given access to personal data, the
purpose of the data sharing, categories or personal data
concerned, intended recipients, rights as data subjects, and
other information that would sufficiently notify me/us of
the nature and extend of data sharing and the manner of
processing. Your personal references will not be contacted
unless necessary. Please make sure that your personal
references are informed and contact numbers should be
active.
